I’ve had clients who can pull a respectable deadlift and still stop halfway across the parking lot because both grocery bags are cutting off circulation in their fingers. Strong in the gym, undertrained in the hand. The measurement behind that gap has become a fixture in longevity content lately, and most of what gets said about it deserves more skepticism than it gets.
The Obvious Explanation
Here’s the intuitive read, and it’s a fair one: weak grip is just downstream of weak everything. People with less overall muscle mass and less overall strength are going to test weaker on a hand dynamometer along with every other test, so of course grip correlates with mortality. It’s a proxy that happens to be easy to measure, nothing more.
That explanation has been tested directly. A long-running cohort study adjusted for arm muscle area, BMI, fat-free mass, and percent body fat, and grip strength still predicted mortality after all of it (1). If grip were purely riding on the coattails of general muscle mass, controlling for muscle mass should have made the association disappear. It didn’t. Something in the measurement is picking up more than “how much muscle does this person have,” and the honest answer is nobody has fully pinned down what. The leading hypothesis points toward neuromuscular integrity, how well motor units are firing and recruiting, rather than raw tissue size (2).
So the lazy version of the skeptical take doesn’t quite hold up either. Grip strength isn’t simply a stand-in for overall strength that happens to be convenient to measure.
What the Research Doesn’t Show
Here’s where the popular version of this story gets ahead of itself. The PURE study found grip strength outpredicted systolic blood pressure for mortality risk across 17 countries (3). The Tromsø Study found the pattern held from midlife onward over 17 years of follow-up (4). Both are real, well-run cohorts. Neither one, nor any study like them, tested if training grip strength changes mortality. They’re observational. Weaker grip correlates with dying sooner. That is not the same claim as strengthening your grip will help you live longer, and most of the content repeating these studies quietly slides from one claim to the other without anyone noticing the jump.
Nobody has run that trial. Until someone does, the responsible position is that grip strength is a marker worth paying attention to, not a lever with a known effect size.
The Practical Position
Train grip for the actual tasks it serves, not for a number from a study you’re trying to game. Loaded carries, farmer carries, suitcase carries, ask the hand to hold real weight over real distance. Dead hangs build the same capacity in a different position and expose weak wrists and shoulders fast. Thick-grip or fat-grip work forces the hand to work harder at the same load, which matters because daily life doesn’t hand you a nicely diametered barbell to grab.
Wrist range matters here too, more than people expect. A hand that can produce force but can’t move the wrist through full flexion and extension is working with less usable strength than the raw number suggests, since grip output changes with wrist position. This connects to why mobility work after 60 isn’t a separate project from strength work, they’re the same system viewed from different angles.
Where This Leaves You
Track grip anyway as one honest input into how your training is progressing. It’s a rough, low-effort signal that the rest of the system is moving in the direction you want, hard to fake and hard to improve without doing real work. Hold the claim at the size it earned: a measurement that correlates with almost everything, worth watching, not a lever anyone has proven you can pull to buy more time. That’s the same case made throughout the longevity work here: build real capacity, and let the numbers that track it be evidence, not the goal.
